New Passport Application for a Minor

Requirements:

   Complete our order form

   

2. New U.S. passport application form DS-11 online
   Fill out new passport application form online to avoid processing delays. Printed our form has to be signed in front of an Acceptance Agent by one or both parents. We can complete online application for you at no additional charge – please contact our office for a paper application.
3. Letter of Authorization.We will email you a letter of authorization depending on which passport office will be processing your application, the urgency of your case, etc.
4. One passport-size photo (2"x2", front of the face, plain white background, sharp quality, no glasses or accessories)
5. Proof of US citizenship – one of the following original documents and its photocopy:
   • US Birth Certificate
   • Previously issued undamaged US passport (and Passport Card if you were issued one before)
   • US Naturalization Certificate
     Note: All originals will be returned to you with your new passport.
6. Proof of Identity (for each parent) – a valid government-issued ID and photocopies of its both sides
7. Proof of parental relationship
8. Evidence of travel in the next 14 calendar days – airline reservation, travel itinerary, letter from employer. If you do not have either of these items, please contact our office.
9. Payment of the government fee in the amount of $160 in the form of a personal check or money order check or a money order made out to the US Department of State with your full name, date of birth and address listed on the memo line.
10. If you would like to obtain a Passport Card in addition to the Passport Book, please indicate that on your DS-11 application form and add $15 to the government fees.
11. Consent form DS-3053 from the parent who cannot be present at the sealing of the documents. The form needs to be notarized, properly dated and accompanied by photocopies of both sides of that parent's valid government-issued ID.

If the previously-issued passport was lost or stolen, please indicate that when completing the DS-11 form, which will generate an additional form DS-64 when you complete the process online.

If the previously-issued passport is damaged, please provide a brief written explanation of how and when it was damaged.

Getting the Documents Authorized

The documents listed above will need to be brought to a local Acceptance Facility (post office, courthouse, government center) and processed by an Acceptance Agent. The applicant (the minor) has to be present and accompanied by both parents, unless one of the parents provides consent form DS-3053.

Authorization Procedure

The Acceptance Agent will verify your identity, witness signing of your application and seal all documents in an envelope that will then need to be brought or sent to our office, so we could hand-deliver it to the State Department to expedite your application. Acceptance agent fee is $35.00, payable directly.

The Acceptance Agent should write the applicant's full name, departure date, and date of birth on the outside of the sealed envelope. If two or more applications are sealed in the same envelope please make sure that the information about both applicants is written on the envelope.

To avoid any confusion, you should inform the Acceptance Agent that you are using an agency (aka Third Party, aka Hand-Carry service) to expedite passport processing and that your documents need to be sealed to be later hand-delivered to the State Department by the Third Party. The policy is detailed in the Acceptance Agent Manual.

We must receive your sealed envelope very promptly, as it has to be submitted to the State Department within a week after it is sealed.
